This document is certified by the University Registrar. You may request multiple copies of this document. The University charges P40.00 per copy. You may claim the document after three working days of processing. If you are unable to claim the document personally, you may authorize a representative in your behalf. In case of representative, please present letter of authorization along with valid ID’s.
